UF Health Engages Patients on Advance Directives

Companies mentioned in this article: Emmi Solutions

GAINESVILLE, FL -- (Marketwired) -- 07/07/14 -- UF Health Shands Hospital is empowering their patients to understand the importance of Advance Directives and the reasons for having them in place. Through the use of Emmi Solutions' personalized and interactive multi-modal program, patients are engaged to plan ahead for their future health needs and the challenging decision making that comes with it. Through preparation, physicians and loved ones are informed about their end-of-life wishes and prepared to act.

Patients are welcome to share their reactions after viewing the program. One person said: "Excellent presentation of very useful information. I have been putting this conversation off with a family member, but it's time. I think I will fill one out for myself and share it with her to start the conversation about her wishes." Another stated, "It was very helpful. I have made advance directives, but this review reinforced my decisions and made me comfortable."

"By facilitating these essential but challenging conversations, we're supporting our patients and their families through the entire continuum of care," said Anthony M. Clarizio, executive director of UF Health Shands HomeCare and ElderCare of Alachua County.

The Advance Directives program is part of Emmi Solutions' Advance Care Suite, including Hospice and Palliative Care programs, which supports future health and end-of-life planning. The suite provides empathetic, understandable and actionable information to support sensitive conversations between patients and providers.

"Patient-centered care means proactively giving patients meaningful information when they are still able to use it to make the best decisions for themselves," said Anne M. Meiring, senior quality improvement specialist with the patient experience program at UF Health Shands Hospital.

"Critical communication gaps can exist during end-of-life healthcare and they contribute to the emotional and financial burdens that people, their families and their caretakers experience during such a trying time," said Devin Gross, CEO of Emmi Solutions. "Empowering people to make decisions about their own care can help to ease some of these strains."

Emmi® programs utilize simple language and compelling visuals to make sense of complex medical information. Select programs act as decision aids to support Shared Decision Making by capturing goals and preferences and explaining risks and benefits associated with treatment options. The web-based programs can be shared with family and friends, guiding individuals to have meaningful conversations about their treatment wishes with all involved in the process. The Advance Directives program was also recently awarded the Gold award in the annual Web Health Awards.
